	This routine builds a parameter page subpage description string.

	index_page_name	null terminated ASCII index page name (eg "M4")
	level_1_name	null terminated subpage level 1 name
	level_2_name	null terminated subpage level 2 name
	level_3_name	null terminated subpage level 3 name
	param_page_text	returned concatenated parameter page text
			(format is pppp\11\2x\3x where pppp is the index
			 page name (2-4 characters), 11 is the first level
			 sugpage name, 2x is the second level subpage name,
			 and 3x is the third level subpage name)

	This function returns ACNET status values as follows:

	OK			success
	CLIB_INVARG		invalid page name or subpage indices
	CLIB_INVTYP		not a parameter page file
	SQL_xxx			error reading database information

	This function requires the following include files:
