					  index_page_name.i1a.r,
					  level_1_index.i4.r,
					  level_2_index.i4.r,
					  level_3_index.i4.r)

This routine builds a parameter page subpage selection dialog.

top_row top row of window (use WMNGR_CENTER for centering
or WINDOW_xxx_COORD macros in macro for special
